What is Case Management?

Case Management is a collaborative process involving planning, facilitating, and advocating for services to meet an individual's holistic needs. By actively involving the member, we prioritize their preferences and goals, creating a tailored support system that enhances their quality of life and fosters independence.

Person-Centered and Goal-Oriented Approach

We develop personalized plans outlining the member's goals and steps needed to achieve them. This approach ensures our services align with what truly matters to the individual, promoting active participation and engagement.

Coordinating Support

We coordinate a comprehensive range of support, including natural supports (family and community networks) and paid services (therapy, medical care, and educational assistance). This holistic approach addresses the individual's diverse needs, creating a robust support network.

Ensuring Accessibility, Accountability, and Continuity

We strive to make services readily available to our members, minimizing barriers to access. Our commitment to accountability ensures transparency in our actions. Continuity of care is critical; our Case Managers provide consistent support throughout the individual's life, adapting the support plan as needed.
