Facility-Based Day Habilitation

The Facility-Based Day Habilitation program at Westbrook Health Services is a structured initiative designed to engage members in meaningful and productive activities outside their residential homes. Our program focuses on promoting the acquisition or maintenance of essential skills, ensuring that each activity aligns with the individual's personal strengths, needs, wishes, desires, and goals. Guided by the principles outlined in the West Virginia Bureau for Medical Services, Chapter 513 Intellectual and Developmental Disabilities Waiver (IDDW), our services are person-centered and goal-oriented.
 

Person-Centered Approach

Our Facility-Based Day Habilitation program is built on a foundation of person-centered planning. Each member's unique strengths, interests, and aspirations are considered when designing their daily activities. By focusing on what truly matters to each individual, we create a supportive environment that encourages personal growth and skill development. Our goal is to empower members to lead fulfilling lives, actively participating in their communities and achieving their personal objectives.

The activities offered in our Facility-Based Day Habilitation program are not only meaningful but also productive. Pre-Vocational Services

Westbrook Health Services also offers Pre-Vocational services aimed at preparing individuals for integrated community-based employment. These services are designed to create a path to meaningful employment where individuals are compensated at or above minimum wage. Our Pre-Vocational services focus on helping each member attain the highest level of work suited to their strengths, interests, priorities, and abilities. By providing tailored support and training, we enable individuals to pursue fulfilling careers in inclusive workplace settings.
