Person-Centered Support

At Westbrook Health Services, we offer Person-Centered Support to adults aged 18 and older. This service is provided in either an Unlicensed Residential Home or a Licensed Group Home, ensuring a safe and supportive environment tailored to each individual's unique needs. Our approach is designed to promote independence and community participation, enhancing the quality of life for each member.
 

Dedicated Direct-Support Professionals

Our Person-Centered Support is delivered by awake and alert Direct-Support Professionals who do not live in the home with the members. These dedicated professionals are available around the clock to provide assistance and support. Their primary role is to offer individually tailored training and support activities that help members develop essential life skills and achieve their personal goals.

The support activities provided by our Direct-Support Professionals are customized to meet the specific needs and preferences of each member. These activities are designed to:

Enhance Daily Living Skills: We assist members in developing skills necessary for everyday tasks such as cooking, cleaning, personal hygiene, and managing finances.


Promote Community Participation: We encourage and facilitate members' involvement in community activities, helping them build social connections and participate fully in their community.

Support Personal Goals: Whether a member's goal is to find employment, pursue education, or develop a new hobby, our team provides the necessary support and resources to help them succeed.

The ultimate goal of our Person-Centered Support program is to enable members to live independently and inclusively within their community. By focusing on individual strengths and preferences, we create an environment where members feel empowered and supported to lead fulfilling lives.
